1. A consolidation engine to consolidate health information that is under regulatory control and is from healthcare entities and patients, the consolidation engine comprising: one or more network interfaces configured to provide access to the Internet; one or more processors coupled to the one or more network interfaces to access a plurality of data sources through the Internet, wherein the plurality of data sources corresponds to two or more of a healthcare provider and/or a healthcare payer, the one or more processors to execute instructions to: access a first set of confidential medical information for an identified patient, wherein the first set of confidential medical information: came electronically from a first data source of the plurality of data sources; and includes one or more of: a first indication of a health condition of the identified patient; and/or a first indication of a health care service provided to the identified patient; access a second set of confidential medical information for the identified patient, wherein the second set of confidential medical information for the identified patient: is derived from a second data source of the plurality of data sources; and includes one or more of: a second indication of a health condition of the identified patient; and/or a second indication of a health care service provided to the identified patient; consolidate the first set of confidential medical information for the identified patient and the second set of confidential medical information for the identified patient to form a composite set of confidential medical information for the identified patient, wherein consolidating the first set of confidential medical information for the identified patient and the second set of confidential medical information for the identified patient comprises comparing the first set of confidential medical information for the identified patient to the second set of confidential medical information for the identified patient; store the composite set of confidential medical information for the identified patient; and store authentication information for a user that is approved for access to at least part of the composite set of confidential medical information for the identified patient; select a first item from the composite set of confidential medical information for the identified patient; select a second item from the composite set of confidential medical information for the identified patient, the second item relating to the first item; characterize reliability of the first item and the second item from the composite set of confidential medical information for the identified patient based at least in part on a correspondence of the first item and the second item to a preventive care recommendation, wherein the characterizing comprises comparing the first item with the second item and identifying a discrepancy based at least in part on the comparing the first item with the second item; assign a numeric score corresponding to first item and the second item from the composite set of confidential medical information for the identified patient based at least in part on the characterizing reliability, wherein the numeric score is a function of the correspondence of the first item and the second item to the preventive care recommendation; and provide a prompt based at least in part on the numeric score corresponding to the first item and the second item from the composite set of confidential medical information for the identified patient; and one or more storage media coupled to the processor to retain the instructions; wherein at least one of the one or more network interfaces allows access by the user that is approved for access to at least part of the composite set of confidential medical information for the identified patient.

The consolidation engine of claim 1 , wherein the comparing the first set of confidential medical information for the identified patient to the second set of confidential medical information for the identified patient comprises: comparing the first indication of the health condition of the identified patient to the second indication of the health condition of the identified patient; and comparing the first indication of the health care service provided to the identified patient to the second indication of the health care service provided to the identified patient.

The consolidation engine of claim 1 , wherein: the first set of confidential medical information for the identified patient includes a first item of biographical information of the identified patient; the second set of confidential medical information for the identified patient includes a second item of biographical information of the identified patient; and consolidating the first set of confidential medical information for the identified patient and the second set of confidential medical information for the identified patient further comprises comparing the first item of biographical information of the identified patient to the second item of biographical information of the identified patient.

4. The consolidation engine of claim 3 , wherein the biographical information of the identified patient comprises two or more of a name, a sex, a marital status, an age, a date of birth, an address, a height, and/or a weight.

The consolidation engine of claim 1 , wherein the one or more processors are to execute further instructions to: process identification information received from the user through at least one of the one or more network interfaces; authenticate the user with the authentication information; and allow the user to access at least part of the composite set of confidential medical information for the identified patient.

6. The consolidation engine of claim 1 , wherein the user corresponds to one or more of the identified patient, the healthcare provider, and/or the healthcare payer.

A method to consolidate health information that is under regulatory control and is from healthcare entities and patients, the method comprising: accessing a first set of confidential medical information for an identified patient, wherein the first set of confidential medical information: came electronically from a first data source of a plurality of data sources, wherein the plurality of data sources are accessible through the Internet, and the plurality of data sources corresponds to two or more of a healthcare provider and/or a healthcare payer; and includes one or more of: a first indication of a health condition of the identified patient; and/or a first indication of a health care service provided to the identified patient; accessing a second set of confidential medical information for the identified patient, wherein the second set of confidential medical information for the identified patient: is derived from a second data source of the plurality of data sources; and includes one or more of: a second indication of a health condition of the identified patient; and/or a second indication of a health care service provided to the identified patient; consolidating the first set of confidential medical information for the identified patient and the second set of confidential medical information for the identified patient to form a composite set of confidential medical information for the identified patient, wherein consolidating the first set of confidential medical information for the identified patient and the second set of confidential medical information for the identified patient comprises comparing the first set of confidential medical information for the identified patient to the second set of confidential medical information for the identified patient; storing the composite set of confidential medical information for the identified patient; storing authentication information for a user that is approved for access to at least part of the composite set of confidential medical information for the identified patient; selecting a first item from the composite set of confidential medical information for the identified patient; selecting a second item from the composite set of confidential medical information for the identified patient, the second item relating to the first item; characterizing reliability of the first item and the second item from the composite set of confidential medical information for the identified patient based at least in part on a correspondence of the first item and the second item to a preventive care recommendation, wherein the characterizing comprises comparing the first item with the second item and identifying a discrepancy based at least in part on the comparing the first item with the second item; and assigning a numeric score corresponding to first item and the second item from the composite set of confidential medical information for the identified patient based at least in part on the characterizing reliability, wherein the numeric score is a function of the correspondence of the first item and the second item to the preventive care recommendation; and providing a prompt based at least in part on the numeric score corresponding to the first item and the second item from the composite set of confidential medical information for the identified patient.

The method of claim 9 , wherein the comparing the first set of confidential medical information for the identified patient to the second set of confidential medical information for the identified patient comprises: comparing the first indication of the health condition of the identified patient to the second indication of the health condition of the identified patient; and comparing the first indication of the health care service provided to the identified patient to the second indication of the health care service provided to the identified patient.

The method of claim 9 , wherein: the first set of confidential medical information for the identified patient includes a first item of biographical information of the identified patient; the second set of confidential medical information for the identified patient includes a second item of biographical information of the identified patient; and consolidating the first set of confidential medical information for the identified patient and the second set of confidential medical information for the identified patient further comprises comparing the first item of biographical information of the identified patient to the second item of biographical information of the identified patient.

12. The method of claim 11 , wherein the biographical information of the identified patient comprises two or more of a name, a sex, a marital status, an age, a date of birth, an address, a height, and/or a weight.

The method of claim 9 , further comprising: processing identification information received from the user through at least one of the one or more network interfaces; authenticating the user with the authentication information; and allowing the user to access at least part of the composite set of confidential medical information for the identified patient.

One or more non-transitory machine-readable media having machine-readable instructions thereon which, when executed by one or more processors, implements a method to consolidate health information that is under regulatory control and is from healthcare entities and patients, the method comprising: accessing a first set of confidential medical information for an identified patient, wherein the first set of confidential medical information: came electronically from a first data source of a plurality of data sources, wherein the plurality of data sources are accessible through the Internet, and the plurality of data sources corresponds to two or more of a healthcare provider and/or a healthcare payer; and includes one or more of: a first indication of a health condition of the identified patient; and/or a first indication of a health care service provided to the identified patient; accessing a second set of confidential medical information for the identified patient, wherein the second set of confidential medical information for the identified patient: is derived from a second data source of the plurality of data sources; and includes one or more of: a second indication of a health condition of the identified patient; and/or a second indication of a health care service provided to the identified patient; consolidating the first set of confidential medical information for the identified patient and the second set of confidential medical information for the identified patient to form a composite set of confidential medical information for the identified patient, wherein consolidating the first set of confidential medical information for the identified patient and the second set of confidential medical information for the identified patient comprises comparing the first set of confidential medical information for the identified patient to the second set of confidential medical information for the identified patient; storing the composite set of confidential medical information for the identified patient; storing authentication information for a user that is approved for access to at least part of the composite set of confidential medical information for the identified patient; selecting a first item from the composite set of confidential medical information for the identified patient; selecting a second item from the composite set of confidential medical information for the identified patient, the second item relating to the first item; characterizing reliability of the first item and the second item from the composite set of confidential medical information for the identified patient based at least in part on a correspondence of the first item and the second item to a preventive care recommendation, wherein the characterizing comprises comparing the first item with the second item and identifying a discrepancy based at least in part on the comparing the first item with the second item; and assigning a numeric score corresponding to first item and the second item from the composite set of confidential medical information for the identified patient based at least in part on the characterizing reliability, wherein the numeric score is a function of the correspondence of the first item and the second item to the preventive care recommendation; and provide a prompt based at least in part on the numeric score corresponding to the first item and the second item from the composite set of confidential medical information for the identified patient.

The one or more non-transitory machine-readable media of claim 17 , wherein the comparing the first set of confidential medical information for the identified patient to the second set of confidential medical information for the identified patient comprises: comparing the first indication of the health condition of the identified patient to the second indication of the health condition of the identified patient; and comparing the first indication of the health care service provided to the identified patient to the second indication of the health care service provided to the identified patient.

The one or more non-transitory machine-readable media of claim 17 , wherein: the first set of confidential medical information for the identified patient includes a first item of biographical information of the identified patient; the second set of confidential medical information for the identified patient includes a second item of biographical information of the identified patient; and consolidating the first set of confidential medical information for the identified patient and the second set of confidential medical information for the identified patient further comprises comparing the first item of biographical information of the identified patient to the second item of biographical information of the identified patient.

20. The one or more non-transitory machine-readable media of claim 17 , wherein the biographical information of the identified patient comprises two or more of a name, a sex, a marital status, an age, a date of birth, an address, a height, and/or a weight.

21. The consolidation engine of claim 1 , wherein the one or more processors are to execute further instructions to: determine whether the reliability of the item from the composite set of confidential medical information for the identified patient needs improvement.

The consolidation engine of claim 21 , wherein the one or more processors are to execute further instructions to: responsive to a determination that the reliability of the item from the composite set of confidential medical information for the identified patient needs improvement, prompt for further information corresponding to the item from the composite set of confidential medical information for the identified patient.

The consolidation engine of claim 21 , wherein the one or more processors are to execute further instructions to: responsive to a determination that the reliability of the item from the composite set of confidential medical information for the identified patient needs improvement, access at least one of the data sources to seek further information corresponding to the item from the composite set of confidential medical information for the identified patient.
